Traveling Electronic Documentation (TED)

All disciplines of staff at the three sites of Halton Healthcare document electronically in the inpatient areas.  Several outpatient areas are using the e-doc system as well.

Support of the new clinical initiatives include additional online protocols/guidelines and policies, optimizing current functionality to meet documentation and workflow needs. Integration of devices providing patient data is planned for the future. Revisions, refinement and streamlining of the current electronic documentation system are based on input from staff and physicians. HHS follows documentation by exception methodologies and recording to defined standards.

The combined Documentation and TED Advisory Committees have evolved into dynamic committees supporting interdisciplinary communication and documentation transformation.

Orders for patients are entered via the Meditech OE system for consults, labs, tests and procedures. Results are fed into the EMR (Enterprise Medical Record).

HHS continues to build the electronic patient record which is viewable by staff and physicians via the EMR from multiple computer desktops.

Patient care is further enhanced by portal technology. This is a joint initiative with partner hospitals:   HHS, Credit Valley Hospital and William Osler Health Centre. The Reach portal incorporates patient data and provides staff and physicians with key patient information about care and treatment across organizations.

Staff are provided with numerous electronic tools to support clinical decision making – these include e-CPS, Up-to-date, Micromedix to name a few.
