Clinic Location in the Hospital:
We are located on the second (2nd) floor of the hospital, next to the Eye Clinic.
Enter the hospital through the main entrance and take the main elevators to the second (2nd) floor. Once you reach the second floor follow the signs for the Pre-Admission Clinic and the Eye Clinic.
As you exit the elevator, make a right. At the next intersection make a left and then another left. Continue down the corridor straight ahead until you reach a corridor on your right. Follow this corridor to the end, past the Eye Clinic. The Pre-Admission Clinic is located just past the desk for the Eye Clinic on the left-hand side.

Upon your arrival in the Pre-Admission Clinic, please enter the clinic office and check in with the volunteer. They will ask for your Pre-Admission Clinic Package, Ontario Health Card, and your green hospital card (if you have one) please make sure you have these available. Have a seat in the waiting area. When the nurse is ready, she will call for you.
Hours of Operation:
The Pre-Admission Clinic is open for appointments from 7am - 3pm, Monday to Friday.
Once you know your surgery date, please call to book your Pre-Admission Clinic appointment. The appointment booking desk is open Monday to Friday 9am to 5 pm. When you call to book your appointment, please ensure you have; a pen, your Pre-Admission Package, and your calendar/schedule readily available.
Please Note: You must attend the Pre-Admission Clinic 7 - 14 days BEFORE your surgery date.
