Acute Hand Program
This program provides a team approach using physiotherapists and occupational therapists with specialty training to treat complex injuries and post-operative surgeries. Patients are referred by their doctor and may have injured themselves accidentally, been involved in a motor vehicle accident or be part of a workers compensation claim. The program uses a wide variety of treatment techniques and the frequency of therapy is dependent upon the client's needs. Reports that outline a person's functional ability post injury, which may be required for insurance adjusters or lawyers, are done on a fee-for-service basis utilizing standardized assessments. The Asthma Education Centre allows asthma sufferers to take control of their condition and optimize the quality of their life. The centre empowers asthma sufferers through knowledge, monitors the effectiveness of asthma medications, identifies triggers and generally gives the patient a better understanding of their condition. The Audiology service is available in the Rehabilitation Department at the Milton hospital. The program provides a full range of services including hearing tests, hearing aid evaluation, auditory brain stem response and hearing aid prescriptions.Training in the use of hearing aids, counselling and annual follow-up is provided.Acute inpatient service is available to all inpatients with a physician referral. The department also runs the Halton Hearing Aid Clinic. Hearing aids, hearing aid accessories and devices to assist with the management of hearing impairments are available through the dispensary. Cardiac Rehabilitation Program
This program provides rehabilitation and education to individuals with coronary artery disease. Participants include those who have had angina, a heart attack, heart surgery and congestive heart failure. Through exercise and education, this program improves quality of life and life expectancy.
In March of 2001, the Ministry of Health and Long-Term Care announced the creation of a Cardiac Rehabilitation Pilot Project. The project was undertaken with the intent to increase and improve access to cardiac rehabilitation services in Ontario. Halton Healthcare Services received funding for a one-year period and has been designated as one of six Regional Coordinating Centres which involves:
- Planning and Program Development
- Quality Management
- Research and Education
- Outreach activities

ConnectCARE
ConnectCARE is a personal response service that offers help at the touch of a button, 24-hours a day, after a patient leaves the hospital. In and around the home, if a situation occurs where the client cannot reach the phone to call for help, a simple push of a small portable waterproof button will put them in touch with trained monitoring staff through a speaker in the home unit. The ConnectCARE program is ideal for older adults living alone, those with a medical or physical condition or caregivers to at-risk family members. Falls Intervention Clinic
This free service is available to seniors, 65 years of age and older, who are community dwelling and have experienced one or more falls in the past year. Participants will receive a comprehensive assessment by a registered nurse, and a care plan outlining recommendations that will assist in reducing their risk of falls. These may include referrals to physiotherapy, the use of walking aids, home safety measures, and/or a plan to increase physical activity. Clients will be followed up at three, six and twelve month intervals. This service was added in the Spring of 2001 due to a four year grant awarded to the Trauma Prevention Council of Central West Ontario by The Ontario Trillium Foundation. The clinic is a component of the "Are You In Jeopardy? Seniors' Falls Prevention Project". Other activities will include the promotion and distribution of a home safety checklist; the implementation of the "Stepping Out Safely" seniors' pedestrian safety program; and "Community Safety Audits". Halton Diabetes Program
The Halton Diabetes Program provides information, counselling and support services for adults with diabetes and their families. The program aims to help people with diabetes learn to make healthy lifestyle choices and offers continuing support. Through education, the program reduces the risk of and delays the progression of complications. Individual and group programs as well as ongoing follow-up, provided by a team of specialized diabetic educators, help to increase knowledge and alter the management plan as needed. QuitCare
QuitCare is a smoking cessation program designed for people who want the best possible chance to succeed at quitting. The program helps deal with the psychological side of smoking by establishing research-proven skills and techniques. At the same time, physical dependence can be held in check by a nicotine replacement patch. QuitCare is offered in two formats:
- The group format is delivered face-to-face over six 90 minute sessions
- The telephone format is delivered individually over four 45 minute sessions
The program helps the client understand when and why they smoke and uses this information to customize a set of skills to effectively intercept smoking triggers. The final sessions show how to prevent relapse, deal with stress and address other threats to smoke-free status. For more information, please call (905) 338-4691.
Respiratory Rehabilitation Program
The Respiratory Rehabilitation Program is a progressive exercise program for respiratory clients who have decreased tolerance and endurance due to an exacerbation of their Chronic Obstructive Lung Disease or asthma. The program involves a series of group support and education sessions to help patients acknowledge, accept and manage their condition and trains them to continue on a home program once they have completed the Program. Family members are welcome to participate. The program involves an initial assessment, daily attendance for one month with decreasing attendance as the patient's progress plateaus and prepares for discharge to a community program.

Speech Language Pathology
Speech Language Pathology provides adult outpatient services addressing speech, language, articulation, voice and fluency disorders. The service also assesses, treats and consults for swallowing problems.For more information, please call (905) 876-7022.
Work-Fit Total Therapy Centre (WFTTC)
WFTTC has a roster of experienced physiotherapists highly skilled in manual therapy techniques and exercise programs who welcome the opportunity to treat complex cases. Assessment and treatment are covered under various health insurance plans such as extended insurance (benefit plans from places of work), auto and disability insurance and Worker's Compensation (WSIB).
WFTTC is also an approved WSIB Community Clinic which delivers comprehensive return to work treatment programs for injured workers. Work-Fit Total Therapy can provide independent Medical Assessments and Functional Capacity Evaluations (FCE's) for case resolution. For further information, please call 905-876-7022. |
