Oakville-Trafalgar Memorial Hospital Mental Health

The Mental Health program at OTMH provides a comprehensive range of inpatient, outpatient and outreach services to adults, as well as, inpatient services to children and adolescents. For most services, you must reside in Oakville, Milton, Georgetown or Clarkson and be referred by your family physician or psychiatrist. Although services are Oakville-based, guidelines are in place for the safe and timely transfer of medically stable patients from the Milton or Georgetown hospitals.

Inpatient Services

Adult Psychiatric Inpatient Service
This 22-bed, 24-hour service provides psychiatric assessment, short-term treatment and stabilization for adults who cannot be safely or effectively treated on an outpatient basis. The unit includes a four-bed Psychiatric Intensive Care Unit (PICU) designed and staffed to provide added security to high risk patients. For more information, please call 905-338-4633.

Halton Regional Child and Adolescent Psychiatric Inpatient Service
This 10-bed inpatient psychiatric unit provides short-term treatment and stabilization for children and adolescents with acute psychiatric illnesses. Referrals are accepted from family physicians, psychiatrists and community agencies. A multi-disciplinary team of nurses, social workers, a psychiatrist, a teacher, an instructional assistant, child youth workers, an occupational therapist and assistant, and a child life specialist provide assessment, crisis management, family programs and links to follow-up services.

Consult Liaison Psychiatry
Medical/surgical inpatients with mental health concerns can be referred by the attending physician for assessment by a consulting psychiatrist at the Oakville, Milton and Georgetown Hospital hospitals. In addition, specific to the geriatric inpatient population over 65 years of age, Geriatric Psychiatry Nurses provide in-depth assessment and treatment suggestions. Typical reasons for referrals include: new onset of mental illness (depression, anxiety), delirium, cognitive impairment, chronic mental illness and behaviours associated with these illnesses. Geriatric Psychiatry Nurses are available at the Oakville and Milton hospitals.

Central Intake Services
This service provides assessment and treatment recommendations to family physicians. It is the point of entry to most outpatient services. The team consists of a psychiatrist, nurses, a social worker or an occupational therapist.

Child and Youth Outpatient Services
The Child & Youth Mental Health Outpatient Services consists of a community based, multidisciplinary team dedicated to the assessment and short-term treatment of children and adolescents who are experiencing mental health concerns.

The team includes a child and adolescent psychiatrist, a psychiatric nurse, a social worker and an intake worker.

Community Support Program
The Community Support Program provides case management to enable people with severe and persistent psychiatric illnesses to attain and maintain maximum self-sufficiency, stability and integration in community living. A team of nurses and social workers provide long-term slow paced rehabilitation.

Community Treatment Order Program
A Community Treatment Order (CTO) is a signed legal agreement that allows persons with a severe and persistent mental illness to receive treatment, support and supervision in their own community instead of being in the hospital. The CTO Program offers co-ordination and clinical case management. The CTO Coordinator provides consultation with the CTO issuing process, treatment plan development, accessing local community based services and facilitating rights advice. The CTO Case Manager is available for support, monitoring and establishing linkages with community supports and agencies. To contact the program, please call 905-338-4630.

Concurrent Disorders Program
People with a concurrent disorder experience difficulties with both a mental health issue and substance use concern. They often have difficulty finding, understanding and accessing effective treatment. The Concurrent Disorder Program offers a Health and Hope group, a 10 week psycho-educational support group for individuals who have a concurrent disorder. It is the goal of the Health and Hope group to help individuals through recognition and treatment of both their mental health issue and substance use concern simultaneously. Crisis Response Team
The Mental Health Program’s Crisis Response Team provides crisis assessment to Halton Healthcare. Request for crisis consults can be made by Emergency Room(s) Physicians regarding patients/clients who present at the ER(s) with psychiatric/mental health crises. The team is comprised of the on-call psychiatrist and psychiatric crisis nurses and Social Worker.

Eating Disorders Program
This service provides assessment, treatment and education to people of all ages living in South Halton and adolescents to 18 years of age living South Peel. The program offers psychiatric, diagnostic, nutritional and psychosocial assessments. As well, it provides nutritional counselling, individual and family counselling. Geriatric Mental Health Services
The following services are specific to the needs of elderly patients:

Halton Homes Program
Halton Homes Program is a partnership program with Summit Housing & Outreach, Support & Housing Halton, Joseph Brant Memorial Hospital and North Halton Mental Health Clinic. Halton Healthcare provides clinical case management through the Community Support Team for the severe and persistent mentally ill population in supported housing. Treatment includes individual counseling; psychoeducation; family support; linkages to community agencies; liaison with psychiatrists, GP’s, Crisis Units, Crisis management; symptom assessment and management. Visits are in office, home or community. House meetings/case conferences as needed. To contact the program, please call 905-338-4630.

Mental Health Day Program
The The Mental Health Day Program provides rapid intervention for acute psychiatric illnesses. This service provides an alternative to inpatient admission or enables a shortened length of stay in hospital. The focus is on crisis management, symptom stabilization, education, group intervention, individual counselling and the coordination of resources for follow-up after discharge. Patients will be contacted within 48 hours of referral to arrange an admitting appointment.

Outpatient Medication Clinic
This outpatient service provides long-term medication management to adults diagnosed with schizophrenia and other severe and persistent psychiatric illnesses. Clinic psychiatrists and nurses administer medications, monitor symptoms, treat side effects, provide education and support continuing care needs.

Outpatient Counselling Services
This service provides individual counselling for adults with acute or persistent psychiatric illnesses. The focus is short-term therapy.

Phoenix Program Early Intervention
An outpatient, community based program for people experiencing early stages of psychosis who reside in the Halton Region. The Phoenix Program is a clinical program affiliated with Halton Healthcare Services, Joseph Brant Memorial Hospital, North Halton Mental Health Clinic, ADAPT and the Schizophrenia Society of Ontario. Our services include: rapid psychiatric assessment, treatment based on individual concerns/goals including education about psychosis, assistance with work, school, social and leisure activities, specialized support to family members and friends, addictions counselling, and introduction to other community programs and planning for management of illness after discharge from the program. The staff available to support clients include: family educators, nurses, occupational therapists, psychiatrists, social workers, addiction clinicians and peer support/recovery mentors.
