Acute Hand Program
This program provides a team approach using physiotherapists and occupational therapists with specialty training to treat complex injuries and post-operative surgeries. Patients are referred by their doctor and may have injured themselves accidentally, been involved in a motor vehicle accident or be part of a workers compensation claim. The program uses a wide variety of treatment techniques and the frequency of therapy depends on the client's needs.
Reports that outline a person's functional ability post injury, which may be required for insurance adjusters or lawyers, are done on a fee-for-service basis using standardized assessments. Service is available at both the Oakville and Milton hospitals.

Asthma Education Centre
If you or someone in your family has asthma this program is for you. Through education and hands on practice you will learn to take control of your asthma.
The centre provides one-on-one education to children, teenagers, adults and their families. Asthma education is provided by Certified Asthma Educators. The Centre accepts referrals from physicians and other health professionals. As well, patients requiring emergency room care at Halton Healthcare will be referred to the centre. Once the center receives a referral form we will contact you directly to book the next available appointment.
Routine breathing tests will be performed at the beginning of each session for those who are able to perform the test.

Butt Out Smoking Cessation Program
The Butt Out program is specifically designed for smokers who want to quit. The program consists of seven weekly sessions that build on the skill and information needed to develop a comprehensive individualized quit plan and provide support for smokers as they go through the quitting process.
There is no cost associated with the program. Group sessions are available at all three sites of Halton Healthcare Services. For dates and locations of the next available session please contact us at (905) 338-4691.
Cardiac Rehabilitation and Education Program
This program provides rehabilitation and education to individuals with heart disease. Participants include those who have had angina, a heart attack, heart surgery or congestive heart failure. Through exercise and education, this program improves quality of life and life expectancy.
The Cardiac Rehabilitation team includes a medical director, registered nurses, dieticians, physiotherapists, kinesiologists, social workers, and exercise supervisors from the community. Together, this multi-disciplinary team provides medical evaluation, counseling, and supervised exercise classes.
It is a six-month program during which patients attend exercise classes twice a week. Individuals benefit from programs specifically designed to meet their needs. Along with exercise, education about secondary prevention of heart disease is a core component of the program. There are monthly educational workshops that cover topics such as nutrition, medication and risk factors.
The Ministry of Health and Long-Term Care has taken important steps in providing a comprehensive and coordinated system of cardiac rehabilitation for heart patients in Ontario. In 2001, HHS received funding to test the Ministry model for the provision of cardiac rehabilitation services. HHS was designated a regional coordinating site for the Halton-Peel area. Through this initiative, the Ministry evaluated and assessed cardiac rehabilitation services in Ontario and provided strategies for improving patient access across the province.

Central West Eating Disorder Program
The Outpatient program at Halton Healthcare Services is located in Oakville and offers services to people of all ages living in South Halton, and adolescents up to 19 years, living in South Peel. The program serves those individuals and their families who are affected by Anorexia Nervosa, Bulimia Nervosa, or Binge Eating Disorder.
The treatment program offers several services including Psychiatric assessment and follow up; Nutrition Assessment and Counselling; Psychosocial Assessment; Family Education and Individual Counselling. The group programs include Adult Psychoeducation; Family Psychoeducation; Skill Building Group and a Body Image Group.

ConnectCARE
ConnectCARE is a personal response service that provides immediate access to help from the home, 24-hours a day. Designed to enhance and support independent living, our service benefits anyone living with a medical or physical condition, active older adults who live alone, as well as those recovering from a hospital stay.
When help is needed, a simple push of a small waterproof button, worn on the wrist or as a pendant, will put the client in touch with our trained monitoring staff. Our hands free, two way speaker unit, attached to the home telephone line, provides voice connection to determine the level of help needed, be it a neighbour, family member of Emergency Services. Falls Interventions Program
This free service is available to seniors, 65 years of age and older, who are community dwelling and have experienced one or more falls in the past year. Participants will receive a comprehensive assessment by a registered nurse, and a care plan outlining recommendations that will assist in reducing their risk of falls. These may include referrals to physiotherapy, the use of walking aids, home safety measures, and/or a plan to increase physical activity. Clients will be followed up at three, six and twelve month intervals. This service was added in the Spring of 2001 due to a four year grant awarded to the Trauma Prevention Council of Central West Ontario by The Ontario Trillium Foundation. The clinic is a component of the "Are You In Jeopardy? Seniors' Falls Prevention Project". Other activities will include the promotion and distribution of a home safety checklist; the implementation of the "Stepping Out Safely" seniors' pedestrian safety program; and "Community Safety Audits". Footcare & Orthotic Centre
The Oakville Hospital Footcare & Orthotic Centre offers a wide range of services which includes the following:
- Foot assessments and recommended treatment plans.
- Custom made foot orthoticss for both casual and dress shoes.
- General footcare for nails, calluses, corns and ingrown toe nails.
- Complex care for high risk patients with foot problems or wounds complicated by diabetes, arthritis, poor circulation and neuropathies.
- Plantar wart treatments.
- Provision of footcare products such as moisture creams, assistive devices, protective padding and specialty socks for diabetics.
- Sale of quality othopaedic and comfort footwear.
- Sale of medical grade compression stockings.
- Reflexology services.
The Footcare and Orthotic Centre is located in Oakville at 461 North Service Road West, Unit 27B.
The centre is open from 8:00 am to 5:00 pm Tuesdays, Wednesdays and Fridays with extended hours Mondays until 6:30 pm and Thursdays until 7:30 pm.
A physican referral is not necessary at The Footcare and Orthotic Centre. Please note that these services are not covered by OHIP. Call 905-338-4669 to make your appointment today.

Fracture Clinic
The Fracture Clinic operates to serve the needs of patients with orthopaedic injuries or problems. Patients are referred to the clinic either through their family physician or following treatment in the Emergency Department. Appointment times are given for the Fracture Clinic in an attempt to minimize waiting time. The visit time varies according to the type of treatment and related tests or x-rays. Other factors, such as the volume of patients requiring care, emergency cases and room availability can impact your overall length of stay. On average, patients can expect to wait approximately 90 minutes. Some of the items that are recommended in the clinic such as fibreglass casts, splints and crutches are chargeable. Do not hesitate to discuss this with the clinic staff. Please note there is no charge for plaster casts.

The Geriatric Assessment Clinic aims to optimize the health and well being of our elderly patients. A specialized team assesses and recommends a plan of care to support the independence and enhance the quality of life of the older person.

Halton Diabetes Program
The Halton Diabetes Program promotes diabetes self-care by providing education and support to adults with diabetes and their families. The program aims to help people with diabetes learn to make healthy lifestyle choices. Through education, the program strives to reduce the risk of and delay the progression of complications. Individual and group programs as well as ongoing follow-up, are provided by a team of specialized diabetes educators. They work with people who have diabetes, and their doctors, to help themselves increase knowledge and skill in diabetes management.

Heart Function Clinic
Heart failure is the most common reason for hospital admission and readmission. The Heart Function Clinic at OTMH, open since January 2000, educates and monitors patients with heart failure after they are discharged from hospital. The clinic teaches patients and their families how to live with heart disease with the goal of maximizing the quality of their life. The clinic is one of several heart function clinics opening across Canada that will be a part of the national congestive heart failure network.
Patients must be referred to the clinic by their cardiologist.

Hip Saver Program
The Work Fitness Plus Physiotherapy Clinic at Oakville Trafalgar Memorial Hospital is offering the Hip Saver Program for those with osteoarthritis, preparing for or recovering from hip replacement surgery. The Clinic has designed an exercise program of special medical exercises and stretches to increase the mobility and strength of the hip and to prevent further deformity.

Home Oxygen Program
The Community Alliance of Integrated Home/Hospital Resources (CAIHR), a partnership between OTMH and Canadian Home Healthcare Inc., is a Ministry of Health approved Oxygen Vendor. The Registered Respiratory Care Practitioners (RRCP) on staff at OTMH provide quality respiratory care, assessment, rehabilitation and patient education for persons requiring long term home Oxygen Therapy. The program is committed to providing a co-ordinated team approach and is designed for clients whose condition is stabilized and treatment regime is optimized. Psoriasis Clinic
The Psoriasis Clinic is a seasonal (10 months per year) service for the treatment of psoriasis. Two clinic dermatologists direct the treatments using ultraviolet light. Education is also an important part of the clinic. The service uses UVA and PUVA techniques.
Pulmonary Rehabilitation and Education Program
This program provides treatment for patients with chronic obstructive lung disease. The program operates three days a week. The program goal is to avoid unnecessary hospital admissions and to improve quality of life by incorporating a prescribed and measured exercise and education program.
Renal Dialysis Unit Running six days a week, three shifts a day, our 24-station Renal Dialysis program, currently serves 140 patients in the Halton area. Also part of the program is the Kidney Function Clinic. This clinic provides education and clinical support to patients who have been diagnosed with kidney disease, serving to delay the onset of dialysis. Patients are educated with regard to diet, medications and lifestyle. Two nephrologists provide medical direction within the program.

Sleep Clinic
The Sleep Clinic at OTMH investigates diagnoses and helps to manage a multitude of sleep disorders. The clinic’s hours of operation are from 8:30 p.m. to 6:30 a.m. Patients coming for a sleep test are connected to a wide range of sophisticated monitoring equipment and settled in for the night in one of the clinic’s “bedrooms”. A trained and experienced Sleep Technologist observes patients during the entire night on a TV monitor in the control centre while the diagnostic equipment records a wide range of data. This data is then scored, analyzed and interpreted to generate a comprehensive sleep study report. Patients are referred to the Sleep Clinic by their family physician or a specialist for the purposes of confirming or ruling out a sleep disorder.

Speech Language Pathology
Speech Language Pathology provides adult outpatient services addressing speech, language, articulation, voice and fluency disorders. The service also assesses, treats and consults for swallowing problems.

Work-Fit Total Therapy Centre (WFTTC)
WFTTC has a roster of experienced physiotherapists highly skilled in manual therapy techniques and exercise programs who welcome the opportunity to treat complex cases. Assessment and treatment are covered under various health insurance plans such as extended insurance (benefit plans from places of work), auto and disability insurance and Worker's Compensation (WSIB).
WFTTC is also an approved WSIB Community Clinic which delivers comprehensive return to work treatment programs for injured workers. Work-Fit Total Therapy can provide independent Medical Assessments and Functional Capacity Evaluations (FCE's) for case resolution. For further information, please call 905-845-9540.
