Oakville-Trafalgar Memorial Hospital Rehabilitation & Geriatrics
The Rehabilitation and Geriatric program includes a wide range of inpatient and outpatient services to meet the needs of our community. In addition to providing rehabilitation to patient care units within the Rehab and Geriatric Program, rehabilitation professionals provide therapeutic services for all other HHS patient programs.

Inpatient Components:
Acute Care Rehabilitation The Acute Care Rehabilitation Service provides clinical therapies (Physiotherapy, Occupational Therapy, Speech Language Pathology) to support all acute care patients throughout the hospital. The service provides acute care to patients of all ages with medical conditions varying from straightforward and simple to complex, multi-system problems. A strong clinical team directed by clinical care pathways provide co-ordinated, goal focused therapeutic care which supports early discharge with appropriate outpatient or community support services.
Acute Rehabilitation and Geriatric Inpatient Unit In this 39-bed unit, patients are dressed in street clothes, eat in a communal dinning room and are expected to do as much for themselves in activities of daily living as their conditions will safely allow. Unit admission criteria stipulate that a patient must be medically stable, have at least one rehabilitation goal and the potential for improvement, must be willing and able to participate and understand and follow direction.
Geriatric Assessment and Rehabilitation Service (GAR) Geriatric Assessment and Rehabilitation Service provides comprehensive assessment and short term rehabilitation to the elderly patients with multiple complex medical problems requiring a multidisciplinary approach to treatment.
Geriatric Hip Fracture Program The Geriatric Hip Fracture Program provides rehabilitation to elderly patients admitted with a hip fracture.
Complex Continuing Care Unit This 21-bed Complex Continuing Care unit provides care to patients who require extensive assistance with activities of daily living and frequently have complex medical, functional and congnitive problems. There are also patients who are classified as awaiting alternative level of care (ALC), typically long-term (nursing home) care who require supportive care and reactivation.
Outpatient Components:
For most of our outpatient programs, you require a referral from your family physician or a specialist.
Acute Hand Program This program provides a team approach utilizing physiotherapists and occupational therapists with specialty training to treat complex injuries and post-operative surgeries. Patients are referred by their doctor and may have injured themselves accidentally, been involved in a motor vehicle accident or be part of a workers compensation claim. The program uses a wide variety of treatment techniques and the frequency of therapy is dependent upon the client's needs.
Reports that outline a person's functional ability post injury, which may be required for insurance adjusters or lawyers, are done on a fee-for-service basis utilizing standardized assessments.

Cardiac Rehabilitation and Education Program
The Cardiac Rehabilitation Program is an individualized exercise and educational program, designed to improve heart and health. The program is most beneficial to people who have or have had angina, a heart attack, congestive heart failure, heart surgery, or an angioplasty. The Cardiac Rehabilitation Program includes medical evaluation, graded exercise tests, education, counselling and prescribed exercise. The multidisciplinary team includes a medical director, nurse, physiotherapist, dietitian, kinesiologist, and a social worker. This program is offered at Milton and Oakville sites.
In September 2006, the exercise component of the Oakville site relocated to the YMCA of Oakville. This community partnership has enhanced our services by providing state of the art equipment and an exemplary treatment environment, while promoting the patient’s successful transition into an ongoing exercise program following their graduation from the Cardiac Rehabilitation Program.

Fibromyalgia Program Fibromyalgia is a chronic syndrome characterized by diffuse muscle pain, debilitating fatigue, morning stiffness and non-restorative sleep. These symptoms are wide-spread, and therefore require a broad treatment plan.
Our program is held three afternoons a week for eight consecutive weeks. Monday afternoon classes are two hours and consist of exercises and educational seminars such as: back/neck care; relaxation techniques; stress management; pain management; lifestyle coping skills; sleep hygiene; pacing your energy and information on commonly used medications. Tuesday and Thursday afternoons consist of 30 minute classes in a heated pool where a qualified therapist instructs on stretching and low impact aerobic exercises. At the end of eight weeks, you should not only feel better but will have developed mechanisms to help you deal independently with your condition. There is a charge of $20.00 at the beginning of the program for educational material.
Participants from previous programs have found the pool sessions so beneficial, a regular twice per week (mornings or evenings) fee-for-service pool program is available.
Staff have developed an exercise video tape specifically for those with fibromyalgia which sells for $20.00 (includes taxes) which is available for anyone to purchase.

Footcare & Orthotic Centre The Footcare & Orthotic Centre offers a wide range of services which includes the following:
- assessments for any type of foot pain or discomfort.
- general footcare for nails, calluses, corns and ingrown toe nails
- complex care for high risk patients with foot problems or wounds complicated by diabetes, arthritis, poor circulation and neuropathies.
- fabrication of custom made foot orthotics for both casual and dress shoes.
- provision of footcare products such as moisture creams, assistive devices, protective padding and speciality socks for diabetics.
- sale of quality orthopaedic and comfort footwear.
- sale of medical grade compression stockings.
- reflexology services.

The centre is open from 9:00 a.m. to 5:00 p.m. Monday to Friday.

Geriatric Assessment Clinic The goal of this service is to optimize health and well-being, support independence and enhance quality of life for the older person. A specialized team assesses and recommends a plan of care to improve the overall well-being of the person.

Hip Saver Program The Work-Fit Total Therapy Centre at Oakville Trafalgar Memorial Hospital is offering the Hip Saver Program for those with osteoarthritis, preparing for or recovering from hip replacement surgery.
The Clinic has designed an exercise program of special medical exercises and stretches to increase the mobility and strength of the hip and to prevent further deformity.

Outpatient Rehabilitation Therapy Physiotherapy and Occupational therapy are available on an outpatient basis with a physician referral. Because this is one of the only OHIP covered services of its kind in the area, you may encounter a waiting list. Services are offered to patients whose problem is acute, painful and/or is limiting their function.

Pulmonary Rehabilitation Program This multidisciplinary program provides outpatient services to patients with chronic obstructive lung disease. Through exercise and education, the program aims to avoid unnecessary hospital admission and improve quality of life.

Rehabilitation StepUp Program StepUp offers a multidisciplinary program of therapy mainly for neurological, orthopaedic and geriatric patients. It assists patients and their families to adjust to the return home from the hospital in a shorter time by providing safe management at home in addition to specific therapy. The program also aims to prevent hospital admissions by working with individuals who are at risk for falls. The StepUp programs accepts referrals from the OTMH Rehabilitation and Geriatric Unit, other hospital and area family physicians.

Rehabilitation Store The Health Solution Centre is a retail store operated by the Rehabilitation Department which offers solutions to improve your comfort, safety and independence. The store carries various assistive devices such as long shoe horns, sock-aids, reachers, special eating utensils, homemaking assists, back and neck supports, a variety of hand splints, basic exercise equipment such as wrist/ankle weights, canes, skin protection products and educational materials. The store carries only products that have been tested by the therapists and which they typically recommended to their patients.
The store is located within the Rehabilitation Department at the Oakville Site(on first floor of Hospital) and is open 8:00 am to 4:30 pm during regular department hours on Monday to Friday. Items can be special ordered if they are not normally carried in the store.
The Health Solution Centre operates at the Oakville Site, but most products are also available at Milton Site.

Therapeutic Pool Program The therapeutic pool is kept at 95 degrees and provides an excellent therapy choice for people suffering from arthritic and musculo-skeleto conditions.
Fee-for-service classes are held in the morning and evenings. You require a referral from your doctor which states you are physically able to tolerate the hot temperature of the pool. (Your doctor may advise against it if you have high blood pressure or heart problems.)

Work-Fit Total Therapy Centre (WFTTC) WFTTC has a roster of experienced physiotherapists highly skilled in manual therapy techniques and exercise programs who welcome the opportunity to treat complex cases. Assessment and treatment are covered under various health insurance plans such as extended insurance (benefit plans from places of work), auto and disability insurance and Worker's Compensation (WSIB).
Our WFTTC is also an approved WSIB Community Clinic which delivers comprehensive return to work treatment programs for injured workers. Work-Fit Total Therapy Centre can provide independent Medical Assessments and Functional Capacity Evaluations (FCE's) for case resolution.
